I used the version from synonomy as starting point. Adjusted the timings, split the lyrics into P1/P2 and added missing lines from the overlapping parts. I also made a couple of lines into R notes. It uses this video - unfortunately there's no HQ widescreen version of it.
USDB now supports duet markers (P1, P2, not #P1, #P2), so please keep them in when uploading duets. Also, your cover URL was incomplete (shortening only works for fanart.tv urls).
